HIPAA Notice of Privacy Practices

This Notice describes how The Gatteri Agency may use and disclose your protected health information (PHI) to carry out functions related to health insurance enrollment, plan comparison assistance, and other activities permitted by law. It also describes your rights to access, amend, and restrict the use of your PHI and how you can file a complaint if you believe your rights have been violated.

Uses and Disclosures of Your Protected Health Information

The Gatteri Agency may use and disclose your PHI for purposes such as enrollment in health insurance plans, plan comparison assistance, and other activities related to treatment, payment, and healthcare operations.
